Perianal fissure, common in German Shpeherds.
I have done a lot of research and there are many options including surgery and drugs. None seem optimal.
What is YOUR opinion on this?

Well i REALLY wish i had not been eating my breakfast looking up this disease now... very disgusting. Poor dog. Antibiotics, pain killers and i'm thinking maybe a high in fibre diet so that there is no pressure or at least less pressure on open sores when deficating. Removal of dead tissue (surgery) is mentioned, i think that would be a good course if there is a lot there. Depending on the severity though, if this was an ongoing thing, and it went to the worst of it (inconstanance, continual pain and infection, open holes etc.) I would suggest that the dogs quality of life may be a question How aweful

Not a cure no, but getting rid of dead tissue will help with infection.

I was busy yesterday on FB looking for a vet I could friend who could help my friend and her dog, Star. She was very upset and treatment seemed iffy at best. I found one! And he gave her optimistic advice, and helpful advice as to how to handle the dog. She is happy and grateful, and hopeful for the future.
We will see how it goes. . .

Hello, I would like to give a bit of an update on Alistar, my 5 year old GSD that has just been diagnosed with this dreadful disease. It was a week today that he had a barium enema and this condition was discovered. I am heart sick needless to say. It is hard watching him but he has been placed on a medication called Cyclosporine that he takes once a day and Cephalaxin three times a day, and I must apply an ointment called Animax to the area. He had a great day today, yesterday not so at all. I made the mistake of giving him ice cream the day before and that was SO WRONG. He is eating great, drinking, playing, acting just himself. You would not know he is this ill until you see him move his bowels. He goes back the vet on the 20th and we will determine what to do next. This medication is very expensive. The cyclosporine is 182.99 and hte Cephalaxin is 40 dollars and the ointment was 22 dollars. I pray it gets this in some sort of remission. There is no cure. I will not do surgery from all I have read and speaking to two people who did. Amputation of the tail is not an option either. I do not want this dog to suffer at all. He is so young and I can not think of the alternative, but, his life comes before anything. Thank You All for your concerns.

Keep us updated. What a aweful thing for a pup to have to go thru Best wishes that something will help if not get rid of at least calm the problem.
